Nurses are required to comply with the Board’s statutes and rules that govern the practice of nursing in the Northern Mariana Islands. The Board mails information to you at the address in your record. Therefore, it is important that you notify the Board of a name, address and email change as soon as possible after the change. However, failure to receive information from the Board about the laws and rules that apply to you does not relieve you of the responsibility to comply with these laws and rules.

- Certificate only: Submit this completed form along with the non-refundable fee of $35 for NMI-127: Issuance of Certificate : $35.00. Please indicate your license number which can be found online at nursys.com.
Certificates are applicable only to those who initially passed the NCLEX under the NMI BON. Certificates are not issued to endorsed license. - License & Certificate: Submit this completed form along with the non-refundable fee of $85 for NMI-126: Issuance of License (ID card) : $50.00 and NMI-127: Issuance of Certificate : $35.00.
